A verified petition has been filed by
the petitioner thru counsel wherein he prayed that after due notice,
publication and hearing, the Court issues an order granting the correction
prayed for and directing the Local Civil Registrar of Laoag City and the
National Statistics Office to make or cause the necessary corrections in the
date and place of marriage of parents in the birth certificate of petitioner
from December 22, 1984, Laoag City to March 16, 1989, Kabacan, Cotabato.
Finding the petition to be sufficient
in form and substance, the hearing of the same is set on August 4, 2016 at 8:30
in the morning before this Court sitting at the ground floor of the Marcos Hall
of Justice, Laoag City.
The Local Civil Registrar of Laoag

and hearing, judgment be rendered directing the Local Civil Registrar of Laoag
City to make corrections in the birth certificate of the petitioner by
correcting the date of marriage of his parents appearing thereon from December
22, 1999 to February 8, 1983; and the Civil Registrar General, Philippine
Statistics Authority, Manila to make corrections in the birth record of
petitioner by correcting his name appearing thereon from ANTON JOHN BARENG to
ANTON JOHN BARENG TAYAG; by deleting the words “NOT MARRIED” and enter therein
the date and place of marriage of his parents which should be February 8, 1983
at Dingras, Ilocos Norte; and by correcting his name appearing in the Affidavit
for Delayed Registration of Birth from ANTON JOHN BARENG to ANTON JOHN BARENG
TAYAG.
Finding the Amended Petition to be
sufficient in form and substance, the hearing of the same is set on August 4,
2016 at 8:30 o’clock in the morning before this court sitting at the ground
